To reserve a "slot" or be placed on a call list, please complete applicable portions of this form and mail with a deposit of $46.88 per student to address below (or full payment if preferred). Privately scheduled classes should include a $200.00 deposit. 

Deposits are transferable or refundable with advance written notice, but are non-refundable without at least 24 hours notice to dcsa1996@yahoo.com.  This form can also be hand delivered 24/7 to drop box or office during any "Open House" period.
