Existing members (a class within the past 12 months or a current Freedom Plan) can reduce any pricing below by $31.88 (and include $25.00 deposit instead).

To reserve a "slot", please mail with applicable deposit to the address below. This form can also be hand delivered 24/7 to drop box or office during any "open house" period.

Students registering for more than one class date should include a separate form for each type of class. ($31.88 deposit with first class date and $25 deposit with second class.) Privately scheduled classes should include a $200.00 deposit. 

With at least 48 hours notice to dcsa1996@yahoo.com, deposits are refundable or transferable to another class date. Without at least 48 hours notice, deposits should be considered forfeited and a new registration form and deposit required for re-scheduling.
